Retatrutide vs Semaglutide: How the Mechanism Differs

Semaglutide works through one hormone receptor. Retatrutide works through three, and that changes both the results and the risks.

Direct answer

Semaglutide activates one receptor, GLP-1, which mainly reduces appetite. Retatrutide activates three: GIP, GLP-1 and glucagon, with glucagon thought to add higher energy expenditure. In a 2023 phase 2 trial the highest dose produced 24.2% average weight loss at 48 weeks. But retatrutide is still investigational, not approved by the TGA, FDA or EMA, and online versions are unregulated.

Key takeaways

  • Semaglutide is a GLP-1 receptor agonist; retatrutide is a triple agonist of GIP, GLP-1 and glucagon receptors.
  • The glucagon component is designed to raise energy expenditure, an effect shown in mice and still being studied in people.
  • Phase 2 and phase 3 trials show large average weight loss, with gastrointestinal side effects and a dose-related rise in heart rate.
  • Retatrutide is not approved by the TGA, FDA or EMA, and the FDA says it cannot legally be compounded in the US.
  • Products sold online as retatrutide have been linked to liver injury in Victoria and found to contain the wrong amounts.

Retatrutide gets talked about as "the next Ozempic", as if it were the same drug turned up. It is not. Semaglutide works through one hormone receptor; retatrutide works through three, and one of those three pushes in a direction the others do not. This page explains the mechanism, what the published trials found, and where retatrutide stands with regulators in October 2026.

What does semaglutide target?

Semaglutide is a GLP-1 receptor agonist. GLP-1 (glucagon-like peptide-1) is a gut hormone released after eating; it boosts insulin when blood glucose is high, slows stomach emptying and reduces appetite through the brain. An agonist is a molecule that switches a receptor on, so semaglutide mimics a long-lasting version of that signal. It is an approved medicine: the TGA's Product Information repository lists Ozempic and Wegovy, and both are authorised in the EU according to the EMA medicines database.

What does retatrutide target?

Retatrutide, developed by Eli Lilly under the code LY3437943, is a single peptide that activates three receptors: GIP, GLP-1 and glucagon. The company's 2022 discovery paper in Cell Metabolism describes it as having balanced glucagon and GLP-1 activity with stronger GIP activity.

  • GIP (glucose-dependent insulinotropic polypeptide) is a second gut hormone that also boosts insulin after meals. Tirzepatide combines GIP and GLP-1.
  • GLP-1 is the same pathway semaglutide uses.
  • Glucagon normally raises blood glucose, which sounds like the wrong direction. In obese mice, the Cell Metabolism paper reported that glucagon activity added an increase in energy expenditure on top of the appetite reduction from GIP and GLP-1.

So the core difference is this: semaglutide mainly reduces how much you eat; retatrutide is designed to reduce intake and also raise energy use. Whether the energy-expenditure effect seen in mice matters in humans is still being studied.

Retatrutide vs semaglutide: how do they compare?

The weight figures come from different trials with different populations and lengths, so they cannot be compared head to head. No published trial has compared the two drugs directly.

What did the retatrutide trials show?

The phase 2 obesity trial in the New England Journal of Medicine (2023) enrolled 338 adults. At 48 weeks the highest-dose group had lost an average of 24.2% of body weight, versus 2.1% on placebo. Side effects were mainly gastrointestinal, dose-related and mostly mild to moderate. Heart rate rose with dose, peaking at 24 weeks and then declining. For context, semaglutide's STEP 1 trial (NEJM, 2021) reported a 14.9% average loss over 68 weeks versus 2.4% on placebo.

Phase 3 data are now arriving:

  • Type 2 diabetes. The TRANSCEND-T2D-1 trial (The Lancet, 2026) randomised 537 adults. Over 40 weeks, average weight change was -11.5% to -15.3% across doses versus -2.6% on placebo, with larger HbA1c reductions than placebo. Between 2% and 5% stopped retatrutide because of side effects.
  • Obesity with complications. Lilly's topline results for TRIUMPH-2 and TRIUMPH-3, reported by HCPLive in July 2026, showed average losses of up to 20.8% and 22.6% at 80 weeks. In TRIUMPH-3, 13.5% of people on the highest dose stopped because of side effects, versus 4.8% on placebo. These are company-announced results, not yet peer-reviewed papers.
  • Liver fat. A phase 2 substudy in Nature Medicine (2024) of 98 people with fatty liver disease found large reductions in liver fat, closely linked to weight loss.

Is retatrutide approved by the TGA, FDA or EMA?

No. As of October 2026 it remains investigational:

  • The TGA's Product Information repository returns no documents for retatrutide.
  • The FDA states that retatrutide "cannot be used in compounding under federal law", is not a component of any approved drug and has not been found safe and effective for any condition.
  • The EMA lists no authorised medicine containing it.
  • According to the HCPLive report, Lilly plans an FDA submission in the first quarter of 2027. A submission is not an approval.

What are the risks of retatrutide sold online?

Because it is not approved, any "retatrutide" bought outside a clinical trial is an unregulated product. The record so far is not reassuring:

  • Victoria's Department of Health issued a health alert on 19 June 2026 after six cases of acute liver injury linked to unapproved products labelled retatrutide, noting that contaminants may be contributing.
  • The Alcohol and Drug Foundation reports that testing of retatrutide products found peptide content ranging from around half to almost double the labelled amount.
  • The FDA has warned companies selling retatrutide and similar drugs falsely labelled "for research purposes" or "not for human consumption". A June 2026 Public Citizen report counted 14 FDA warning letters to retatrutide sellers since 2024 and found eight were still selling it as of May 2026.

Clinical trials monitor heart rate, liver tests and side effects in people screened for safety. A vial from a website comes with none of that.

What should you do if you are considering a weight-loss medicine?

  1. Book a GP appointment and be direct about what you are considering.
  2. Ask about approved options, which come with tested dosing, a label and a pharmacist to explain them.
  3. Ask about clinical trials if you are interested in newer drugs; that is the only legitimate way to access retatrutide now.
  4. Get baseline checks such as blood tests and blood pressure before starting any medicine.
  5. Keep the foundations: protein, strength training and daily movement help protect muscle during weight loss.

For the foundations, see how much protein you need per day, how many days a week beginners should lift and walking after meals and blood sugar. Related reading on the peptide market: why peptide storage claims cannot be verified.

FAQ

  • Is retatrutide stronger than tirzepatide?

No head-to-head trial has been published, so nobody can say. Cross-trial comparisons mix different populations, doses and trial lengths.

  • When will retatrutide be available in Australia?

There is no date. Lilly has said it plans to file with the FDA in early 2027, and an Australian application would be a separate TGA process.

  • Why does retatrutide raise heart rate?

The phase 2 trial reported dose-related increases that peaked at 24 weeks and then declined. The cause and long-term significance are still being studied in larger trials.

  • Can I join a retatrutide clinical trial?

Trials are listed on registries such as ClinicalTrials.gov and the Australian New Zealand Clinical Trials Registry. Ask your GP whether any recruiting trial fits your situation.

  • Is compounded semaglutide the same as Ozempic?

Not necessarily. The FDA has received adverse event reports linked to dosing errors with compounded semaglutide, and in Australia the safest route is a prescribed, approved product from a registered pharmacy.
